ACL Is one of the four vital ligament in the knee of the sportsmen. It is a commonly injured during sports leading to instability. Others do get this injury after a fall from two wheeler or slip while dancing or physical activity. Most of the young population will need to undergo surgery to ensure they can return to their sports and other physical activities which they love to do.
A physically active person with good functioning knee before injury is a good candidate for ACL surgery irrespective of age. Generally people below 50 are more likely to be eligible for the reconstruction to prevent the future complications – specially persistent instability leading to injury to other structures like meniscus of the knee which in turn could bring in early degenerative Osteoarthritis.
The ligament needs to be reconstructed using bodies own tissue. There are two well established technique.
BTB (Bone tendon bone) –also available in allograft. Here the graft is taken from the front of the knee – bone with attached tendon of Patella. There is a small operative scar in front of the knee which often is thought to cause mild PF pain.
The other very popular option is Quadrupled hamstring graft – most commonly used technique for all players athletes and active people. The graft is taken from the Hamstring on the inner side of the knee. Small scar of 15mm on the inner aspect of the upper leg which prevents any problem arsing from scar in sports and is cosmetically more acceptable.
Many types of fixation material and techniques are available but gold standard is Bio –absorbable Hydoxy apetite (Bone material) material which dissolves/ostro integrates with time in surrounding bone.
Hospital stay is generally 24 hours or less.
Immediate – Non weight bearing to partial weight bearing mobilisation with brace and crutches is allowed.
Static quads & ankle pumps started day one after the surgery a small stamp sized water proof dressing is put which allow bathing.
